For this assignment, respond to the following:
Now that you have completed six sections of the community needs assessment, you will be preparing and presenting your findings in a community needs report.
Your report will include the following:
Title page
A completed community needs assessment document (weeks 1 – 6 sections with research data in the charts).
Summaries after each section of the community needs assessment (see example)
References can remain after each section or alphabetical order at the end of the report.
3 Report Questions (section 7)
1. Describe the historical and current status of the health care delivery system with a focus on community health nursing practice.
2. Differentiate between Public Health and Community Health, and the roles and responsibilities of the nurse in the community setting and in the public health settings.
3. Evaluate resources that transcend borders for health promotion, intervention, disease prevention, and coordination of care among populations across the lifespan.
Citations and references are also expected when answering the questions.
You will compile and submit all 7 sections of the community needs assessment into one document, including your summaries from each week, and 3 questions in section 7. Citations/references in APA format should be followed. Refer to grading rubric.
