1. Develop a leadership e-portfolio. Refer to the template. 
* Develop a professional vision statement that projects 5-7 years into the future. Keep in mind that vision is an image of the future that can be put into words.
* Identify values and how they relate to your professional vision statement.
* Develop short (1-3 years) and long term (3-5 year) goals. Goals should open with an action verb. Include a leadership position that you wish to attain 3-5 years after completing the DNP/PhD program
* Identify mentors and or professional contacts who can guide you in your leadership development. Include a few sentences describing each mentor and why this person was selected. How often and by which means will you communicate with these mentors?
* Using SMART criteria, identify 2-3 development goals for each of your two short-term career goals and two long-term career goals (There is no need to expand on all of your goals).
*  Specify 2-3 strategies or action plans for each leadership development goal identified.  Indicate how and when they will get you where you want to go. Based on the results of your emotional intelligence assessment, identify an activity/strategy to develop emotional competence that can be used to reach your career goals. Lastly, identify potential barriers to reaching these leadership development goals and plans to reduce these barriers.
* Identify 2-3 indicators for each leadership development goal that will signify how you have achieved that goal. The indicators should flow from the developmental goals and strategies/action plans.
* In the journal portion, specify the time intervals for monitoring the status of your learning strategies/action plans and indicators. Also indicate how you will set aside time to reflect upon the reasons for successes, barriers, and any modifications that might need to be made in the future.
A couple of clarifying notes:  If you are considering leadership in an institutional specific context, please include a couple of sentences with introductory details.  For example, let’s say I want to be a leader on the “Developmental Outcomes” committee in my hospital, just explain what that means…does it focus on developmental outcomes of pediatric patients? Stroke patients? Novice employees?
