With the geriatric population often presenting multiple comorbidities and taking various medications, effective medication management becomes crucial to avoid polypharmacy and potential adverse drug interactions. Given this, interdisciplinary collaboration among geriatricians, pharmacists, physical therapists, and other specialists is paramount.
As PMHNPs, how would you approach medication management for geriatric patients in collaboration with these professionals?
What specific challenges related to the aging population might you anticipate in this collaborative approach, and how would you address them to ensure the well-being and safety of the patient?
